New information has surfaced regarding the death of Limp Bizkit bassist and co-founder Sam Rivers. His body was discovered at his Florida home earlier this month. According to an official incident report, the 48-year-old musician was found unresponsive in his bathroom under distressing circumstances.
The St. Johns County Sheriff’s Department responded to a 911 call on the morning of October 18. The caller, a woman named Keely, reported finding Rivers face down. She immediately began CPR, and deputies continued resuscitation efforts until fire rescue personnel arrived and pronounced him deceased.
The report provides key details from the scene. Investigators noted a cut above Rivers’ eye, possibly from a fall. However, its role in his death has not been determined. A small amount of coagulated blood was also found in the bathroom.
The document also described facial and neck discolouration that can be consistent with a pulmonary embolism. However, the official cause of death remains undetermined, pending a full medical examination and toxicology results.

Keely informed deputies that Rivers had undergone a liver transplant in 2018. He was taking multiple prescribed medications, which may form part of the ongoing investigation.
Rivers was a foundational member of Limp Bizkit, which he co-founded with frontman Fred Durst in Jacksonville, Florida, in 1994. The band issued a heartfelt statement on Instagram following his passing. They called him their “heartbeat” and the “soul in the sound.”
Known for his fluid bass lines and steadying stage presence, Rivers helped define the nu-metal genre. He performed with the band for the last time at the Leeds Festival in Britain in August 2025.
